What goes into the form
- Student name and class
- The activity: date, destination, hours and transport
- Parent name and emergency phone
- Allergies and relevant medical information
- The parent’s signature

Medical information and privacy
Medical information is sensitive. Keep it only with those who need it — the homeroom teacher and the escort — and delete it after the activity if it is no longer needed. The Privacy Protection Law, 1981 applies to it.
